Every week in my South Miami practice, patients come in describing the same cluster of symptoms: exhaustion they can't shake, emotional numbness, loss of motivation, withdrawal from people they love. Their question is almost always the same: "Am I burned out, or is this depression?"
The answer matters because burnout and clinical depression require different treatments. Treating burnout like depression — rushing to medication — often misses the real problem. Treating depression like burnout — just "taking a vacation" — can allow a serious clinical condition to worsen.

Burnout doesn't stay in its lane. Left untreated for months or years, chronic burnout restructures the brain's stress-response systems in ways that generate true clinical depression. Here's the neurobiological pathway:
Chronic stress floods the body with cortisol, initially enabling you to push through.
Sustained cortisol elevation suppresses hippocampal neurogenesis — the brain literally shrinks its memory and emotional regulation centers.
Dopamine and serotonin signaling becomes dysregulated — reward circuits stop responding normally.
The HPA axis (hypothalamic-pituitary-adrenal) becomes dysregulated, producing abnormal cortisol patterns (too high, too low, or both at different times of day).
What began as situational exhaustion has now become a neurobiological disorder: major depressive disorder.
